Jeffrey Zeleznik, DO, studied medicine at the Chicago College of Osteopathic Medicine in Chicago, Illinois. He then completed his residency in family medicine at Riverside Methodist Hospital in Columbus, Ohio. His decision to pursue a career in family medicine was fueled by his profound appreciation for the enduring relationships and trust that can be cultivated with patients as their primary care provider.
I take great pleasure in building meaningful relationships with my patients and crafting personalized treatment plans that cater to their specific needs. I want my patients to feel confident in my ability to provide care for individuals of all ages, cultural backgrounds, sexual orientations, and gender identities. Creating a safe, comfortable, and accepting environment for everyone is my top priority.
Having grown up in Cleveland, Ohio, Dr. Zeleznik recently relocated to Southern California in 2021. Beyond his professional commitments, he enjoys spending time with his golden retriever, going to the beach, and swimming.

What really struck me is that he doesn’t tell me to lose weight and everything else will fix itself as other doctors have said. I absolutely LOVE Dr. Zeleznik! He recently moved from Orange County to Long Beach, yet I will continue to see him as he is quick to respond, willing to listen, kind and professional. He actually helped with my migraines on top of my treatment from my neurologist. I was still having daily headaches, and he discovered that I was having sinus headaches on top of migraines. I had not even consulted him regarding my headaches/migraines because I have a neurologist for that. He somehow had access to my MRI and called me in (I'm guessing because of the MemorialCare system), and discussed this issue. Something no one else has even considered along with my migraine treatments! I wake up most days now without a headache or migraine! Thank you, Dr Zeleznik.

New to the area and looking for a PCP. I was hopeful based on reviews but I will never come back here. Didnt even take the time to review my chart. Dismissed my medical history and refused me a simple blood test I know I need to monitor my condition. I wish I didnt have to advocate for my health this hard but it is what it is.

Impossible to get in contact with. Will not add your name to Mychart so you can message. Been trying for 6 months to get a medical record fixed as there is an error. His office is always on lunch or unable to chat.

He was polite enough enough however he did not examine me did mention that he only had a certain short amount of time to be with each patient which is ridiculous so I can't really think that in future he would have too much worry about my medical care or be able to catch anything that could be possibly detrimental to my house on the plus side as I said he was polite he did refer me to a couple of specialists I do need I will probably have to go to urgent care for my ear infection, the staff was pretty nice and efficient

Very unprofessional. Kept snapping his fingers in face telling me he didn’t have any answers to help me. I don’t snap my fingers at my dog like that.

I felt like he belittled and dismissed my concerns. Im looking for a new PCP. I do not think this office will be a good fit for me.
